Once you have received confirmation of your place, and have received your accommodation address here in Plymouth, you pre-register with this doctors surgery.

Activate the online registration button, and you will be presented with a pre-registration screen entry page. Select or press 'Go' if the selection is already there.

There are two pages to the registration. Please fill out as many fields as you can, as it helps the surgery process the registration.

PLEASE NOTE - Your current address will be your Plymouth address whilst at the University.

Fill out the Patient Information card, but do not post this card. You will notice there is some duplication of information, but please still fill in this information.
